| 설명 | 담석증과 쓸개염의 치료를 위해 쓸개를 외과적으로 제거하는 것. 급성 염증이 치료될 때까지 수술을 연기할 수 있다. 수술 전 심전도검사와 간기능 검사를 해야 한다. 전신마취하에서 쓸개를 절개하고 쓸개관을 결찰하며, 온쓸개관을 찾아 결석을 제거한다. 가장 흔한 합병증은 담도계의 간관 또는 다른 관의 파열이다. 또한 시술 부위의 감염, 출혈, 담즙유출, 황달이 발생할 수 있다. 담낭절제술은 복강경을 이용해서 시행하기도 한다. |

| 설명 | 세균감염으로 인한 쓸개의 염증. 감염 경로는 온쓸개관 하부의 장내세균에서 감염되는 상행성과 문맥 또는 간동맥에서 감염되는 하행성이 있으며, 감염균은 대장균-포도알균-사슬알균-장티푸스균이고, 쓸개염의 발생은 쓸개돌의 존재가 큰 의미를 가지고 있다. 그 발생 경로는 급성으로 발병하여 다른 합병증 또는 만성으로 이행되는 것, 만성으로 발생하여 그대로 계속되는 것, 만성으로 발생하여 그 경과중에 급성으로 다시 돌아가는 것 등이 있다. 합병증으로는 위-샘창자-잘룩창자-배벽 등에 쓸개의 구멍을 통해 쓸개관샛길을 형성하는 쓸개주위염과 쓸개의 천공 또는 쓸개주위 고름집의 파열로 인한 천공성복막염이 있다. 치료는 카타르성쓸개염은 안정을 취하면서 화학요법 등을 행하고, 화농증-괴저성 축농증 및 그 합병증이 있을 때는 화학요법과 조기수술을 행한다. 수술은 쓸개의 적출을 원칙으로 한다. |

| 설명 | 담즙을 일시 저장하고 농축, 배설하는 역할을 하는 주머니 모양의 구조물인 쓸개의 병변이나 변화를 알기위해서 사용하는 특별한 방사선 촬영의 기술. 쓸개는 단순한 X-선 촬영으로는 보이지 않는다. 그래서 쓸개에만 모이고 X-선 촬영시에 희게 나오는 물질을 입으로 먹이면 이 물질이 쓸개에 모여 담낭을 채우므로 X-선 촬영시에 담낭의 모양을 알아 볼 수가 있다. 이런 방법을 경구쓸개조영술(Oral Cholecystography)라 한다. |

| 설명 | 온쓸개관에 돌이 생기는 것. 가끔 황달이 나타나고 오한과 발열이 동반된다. 담석이 쓸개관을 통하여 빠져나와서 온쓸개관에 돌을 형성하거나 쓸개관의 상행성 감염으로 처음부터 온쓸개관 안에서 돌이 만들어지는 경우가 있다. 온쓸개관 안에서 만들어지는 돌은 대장균에서 분비된 효소에 의하여 결합성 빌리루빈이 비결합성 빌리루빈으로 변화되고, 이는 쓸개길에서 분비된 칼슘과 결합하여 칼슘빌리루빈을 형성한다. 이러한 과정이 반복되어 쓸개관돌이 된다. 돌이 쓸개관에서 샘창자로 가는 담즙의 흐름을 방해하기 때문에 빌리루빈의 수치가 증가하여 황달이 발생하기도 하며, 오한과 발열이 동반된다. 심한 경우에는 담즙의 온쓸개관 정체로 인하여 세균 번식에 의한 쓸개길염이 유발될 수 있는데, 쓸개길염은 급성화농성 쓸개관염으로 진행하여 패혈증에 의한 쇼크가 나타날 수 있다. 시간이 경과하면 간고름집이 발생할 수 있고, 결석성 이자염 등을 유발할 수 있으며, 더 진행하는 경우에는 사망할 수도 있다. 그러나 아무런 증세도 나타나지 않는 경우도 있지만, 합병증은 중대한 문제를 일으킬 수 있으므로 증세가 나타나지 않는 경우라도 합병증이 유발되기 전에 쓸개관결석을 제거해야 한다. |
