피임법

수태를 인공적으로 조절하는 일. 전에는 산아 제한, 출산 조절이라고 했으나 조절 대상은 분만이 아니고 수태이기 때문에 수태 조절로 개정되었다. 모체의 건강이나 경제적인 이유 등으로 임신해서는 곤란한 사정이 있을 때 피임을 계속하고, 아이가 필요해지면 피임을 중지하고 임신할 수 있도록 인위적인 수단을 사용해서 수태를 인공적으로 조절하여 산아를 제한하는 것이다. 이미 임신한 것을 인공적으로 중절하는 것은 여기에 포함되지 않는다. 즉, 피임의 실패는 도리가 없기 때문에 바른 피임의 지식을 얻어 적절한 피임법을 선택해서 바르게 시행하는 것이 바람직하다. 수태 조절에 응용되는 피임법은 수정 저지법과 수태 저지법으로 대별되지만 모두 일시적인 피임법이다. 이것은 국가가 주체인 경우에는 인구 정책이 되며, 가정이 주체인 경우에는 가족 계획이 되는 가장 합리적인 수단이다. 실용적인 면에서 분류한 피임법은 다음과 같다. 【배란일 전후의 성교를 피하는 방법】 배란일 전후 수일간을 금욕함으로써 수정을 미리 막는 것으로, 배란일을 예상하는 방법에는 2가지가 있다. ① 오기노식 수태 조절법 : 한국에서 가장 널리 보급되어 있는 방법으로, 월경 주기가 정확하게 돌아오는 여성에게만 적용이 가능하다. 28일형인 여성의 경우 월경 제1일째부터 세어서 제14일째가 되는 날의 전 3일간과 후 2일간이 가장 임신하기 쉬운 날로 보고 이 기간은 성교를 피한다. 이 방법은 월경 주기가 불규칙한 여성의 경우에는 실패의 확률이 크다. ② 기초 체온표를 응용하는 방법 : 매일 기초 체온을 측정해서 기초 체온 곡선이 고온상이 되어 2일이 경과했을 때는 거의 임신하지 않는 것으로 나타나 있어 3일 이후부터 성교를 갖는 방법이지만, 고온상인지 저온상인지를 정하기가 때로는 곤란한 때가 있다. 【남성이 피임 도구나 약품을 사용하는 방법】 남성이 성교 때 콘돔을 써서 수정을 저지하는 방법이 가장 일반적이다. 콘돔은 남성 성기에 씌워서 사용하는 얇은 고무 제품인 점에서 남성이 사용감을 가지게 한다는 결점이 있고, 여성에게는 심리적인 문제를 포함하여 여러 가지 결점이 지적되고 있지만, 실패율이 낮고 일반적으로 쉽게 구할 수 있으며 누구나 사용할 수 있다는 점이 다른 방법에 비해 장점이다. 특히 최근에는 고무의 질이 개량되고 제품의 품질이 향상되어 파손의 우려도 없다. 이 밖에 정자를 만드는 힘을 약화시킬 목적으로 남성에게 피임 약을 쓰게 하는 경우도 있으나, 피임의 목적은 달성했다 해도 가끔 남성의 성욕을 저하시킨다 하여 실용되지 않고 있다. 【중절 성교법】사정 전에 성교를 중단하고 질 외부에 사정하는 방법인데 질 안에 정액을 들여보내지 않도록 해서 임신을 피하는 것을 목적으로 한다. 이 방법은 특별한 기구를 필요로 하지 않으며 어려움 없이 할 수 있는 장점도 있지만, 피임이라는 목적 때문에 성감을 희생하게 되는 경향이 강하다. 더욱이 결정적인 결점은 임신의 위험도 크다는 점이다. 【여성이 기구를 사용하는 방법】① 페서리

분만 시에 신생아가 받는 분만 외상 중 말초신경 장애성 마비. 대개는 팔이나 안면에 나타난다. 팔의 경우는 상완형과 전완형으로 나눈다. 상완형이 가장 많고, 상지의 이완 마비가 특징이며, 지각 마비는 수반하지 않는다. 신생아는 팔꿈치를 뻗은 대로이고, 손이나 손가락은 움직이지만 팔을 위로 올리지는 못한다. 그대로 놓아두면 낫는 경우가 많다. 이와는 반대로 전완형에서는 손이나 손가락이 움직이지 않고, 주먹을 쥐지 못한다. 또 지각 장애도 나타나며, 상완형보다 낫기 어렵다. 또한 상완형에서는 횡격막 마비를 수반하기도 하며, 호흡장애의 원인이 되나, 예후는 역시 양호하다. 안면 마비는 겸자 분만의 경우에 많은데 안면부의 압박으로 인한 말초성 마비이며, 자연 분만에서도 나타난다. 얼굴 한쪽에 일어나며, 눈꺼풀이 감기지 않거나 입 언저리가 틀어진다. 울 때 뚜렷이 나타나며, 자연히 치유되는 경우가 많으나, 더러는 뇌내출혈로 인한 중추성 마비도 있으므로 주의하여야 한다. 분만 마비는 보통 6개월 이내에 낫지만 마비의 정도가 심할 경우는 마사지나 전기요법 등을 행하기도 한다.
