| 설명 | 동맥의 혈압이 높은 것. 일반적으로 말하는 고혈압은 이 동맥성 고혈압을 말한다. 혈압이란 혈관에 걸리는 압력을 말하는 것으로 대개 특이한 설명이 없으면 동맥의 압력을 말한다. 혈압에는 수축기혈압과 이완기혈압이 있다. 수축기혈압이란 심장이 수축할 때의 동맥에 걸리는 압력을 말하고 이완기 혈압이란 심장이 이완할 경우에 동맥에 걸리는 압력을 말한다. 당연히 수축기 혈압이 이완기 혈압보다 높다. 혈압을 말할 때에 120/80이라고 적는 것의 앞의 것은 수축기 혈압을 의미하고 뒤에 적는 80은 이완기 혈압을 의미한다. 고혈압은 주로 이완기의 혈압을 기준으로 정한다. 여러 가지 기준이 있으나 이완기 혈압이 90mmHg이상인 경우를 말한다. 고혈압 자체에 의한 증상은 별로 없다. 다만 고혈압에 의한 합병증과 동맥경화의 촉진에 의해 병발하는 질환에 의하여 증상이 생긴다. 고혈압에 의한 합병증은 대개 혈관에 걸리는 과도한 압력으로 인한 혈관의 손상이 중요한 원인이다. 고혈압은 동맥경화의 주요 유발인자 중의 하나이다. 동맥경화는 동맥의 벽이 두터워지고 동맥이 탄력을 잃어버리는 질환이다. 어떤 동맥이 동맥경화에 걸리면 이 동맥은 두터워진 혈관벽에 의해서 내경이 작아지게 된다. 이렇게 되면 그 동맥으로부터 혈액을 공급받던 장기는 혈액을 적게 공급받게 되고 그 정도가 심하게 되면 그 장기는 결국 손상을 입게 된다. 동맥경화로 인한 질병의 대표적인 예는 심장의 근육에 혈액을 공급하는 심장동맥에 동맥경화의 변화가 오는 심장동맥병이다. 고혈압의 치료에 쓰이는 약물은 다음과 같은 것이 있다. -이뇨제-콩팥에서 수분과 전해질의 배출을 촉진시키는 약물. 체내의 수분의 양을 감소시켜서 혈액의 흐르는 양을 적게하고 또 혈관주위 조직의 수분의 감소로 혈관을 확장시키는 역할(혈관이 있고 그 혈관을 쌓고 있는 조직에 수분이 차 있으면 그 조직이 혈관을 눌러서 혈관이 확장하는 것을 방해한다)로 혈압을 감소시킨다. -칼슘-통로차단제－모든 근육의 수축에는 칼슘이 필요하다. 즉 혈관의 수축에 관여하는 혈관도 역시 수축을 위해서는 칼슘이 필요하다. 칼슘-통로차단제는 근육세포 속으로 칼슘이 들어가는 것을 방지해서 혈관근육의 수축을 방해하고 이로 인해서 혈관의 확장을 유도한다. -알파차단제, 베타차단제-혈관은 자율신경계의 지배를 받는다. 그중에서도 혈관은 교감신경의 영향에 의해서 수축과 이완을 한다. 교감신경의 작용은 그 수용체에 따라 달라진다. 교감신경의 수용체의 종류에는 알파수용체와 베타수용체가 있다. 혈관을 지배하고 있는 교감신경에서 알파수용체는 혈관의 수축을 일으키고 베타수용체는 혈관의 확장을 일으킨다. 그러므로 알파수용체의 작용을 막는 경우에 혈관의 확장을 유도할 수 있고 베타-수용체의 작용을 항진시킬 경우에 역시 혈관의 확정을 유도할 수 있다. |

| 설명 | 고혈압에 의해서 생기는 심장병. 고혈압심장병이라는 진단을 붙이기 위해서는 최소한 다음과 같은 조건이 부합되어야 하는데, 첫째 심장혈관계에 심장병을 유발할 수 있을 만한 다른 병변이 없이 좌심실 비대가 있어야 하며, 둘째 고혈압을 앓았다는 병력이 있어야 한다. 주로 고혈압에 의한 심장병은 초기에는 좌심실이 비후라는 것으로 특징되어진다. 즉 혈압이 높으므로 혈액을 순환시키기 위해서는 그만큼 심장의 혈액을 보내는 힘이 좋아야 한다. 그 힘을 얻기위해서는 심근의 비후가 필요로 하여 좌심실 근육의 비후가 생긴다. 그리고 고혈압이 지속이 될 경우에는 결국 심장이 제 구실을 하지 못하고 펌프로서의 기능을 잃어버리게 되어 심장기능상실에 빠지게 된다. |

| 설명 | 체온조절 중추가 이상을 초래해 체온이 계속 상승하여 지속되고 있는 상태를 말한다. 원인은 다양하며 체온조절 중추의 장애(뇌질환), 조직장애에 의한 것이 있다. 체온은 일반적으로 겨드랑이, 입안, 곧창자에서 재고, 시간은 10분을 필요로 한다. 통상 37℃ 이하(겨드랑이)로, 입안 온도는 이보다 0.1~0.2℃ 높으며, 곧창자온도는 0.2~0.5℃ 높다. 고체온증에서도 동결과 유사한 세포 변화가 나타난다. 즉 혈관내피세포에 손상이 생기고 혈관투과가 증가되어 부종, 수포 등이 형성된다. |

| 설명 | 갑상샘의 기능이 병적으로 증가하여 호르몬이 과다하게 분비되는 병. 혈액 중의 갑상샘호르몬농도가 상승함에 따라 특유의 임상 증세가 생화학적인 변화를 나타내는 것을 일컫는다. 20~50대의 여성에게서 많이 발생한다. 혈액 속에 갑상샘호르몬이 많아지면 물질대사가 항진되므로 몸이 더워지고 외부 온도에 대해 매우 예민해진다. 따라서 더위를 못 견뎌하고 땀을 많이 흘리며 식욕은 증가하는데도 체중이 계속 줄어 대개 1~2개월 사이에 3~4kg씩 빠진다. 맥막이 빨라지고 가슴이 두근거리며 부정맥이 생겨 맥이 불규칙해지기도 한다. 피부는 따뜻하고 습해지며, 목에 이물감이나 통증을 느끼기도 한다. 신경이 예민해지고 불안해하며 늘 피로를 느낀다. 잠을 잘 못자고, 손이 떨리며, 팔 다리의 힘이 약해지고 심하면 마비증세가 나타난다. 월경주기가 불규칙해지며, 양이 감소하거나 월경이 중단되기도 한다. 갑상샘이 있는 목 앞쪽이 튀어나오거나 심한 경우 눈이 튀어 나오는 수도 있다. 이 병은 자가면역병인 그레이브스병에 의한 경우가 대부분이지만, 이 밖에 중독성결절성갑상샘종, 뇌하수체의 갑상샘자극호르몬분비샘종이나 아급성 갑상샘염의 초기에도 나타난다. 항갑상샘제나 방사성 요오드를 복용하여 갑상샘 기능을 억제하거나 심한 경우 갑상샘 제거수술로 치료한다. |
